Chronic Venous Insufficiency (CVI)
Veins are the blood vessels of the body that return blood from the arms, legs, and organs back to the heart. Venous insufficiency is a condition in which the flow of blood through the veins is impaired. Venous insufficiency can be caused by a number of disorders of the veins, particularly deep vein thrombosis (blood clot) or varicose veins.
In healthy veins, there is continuous flow of blood from the limbs back toward the heart. There are valves within the veins of the legs that prevent the backflow of blood. Venous insufficiency occurs when forward flow through the veins is obstructed, as in the case of a blood clot, or if there is backward leakage of blood flow through damaged valves. In many cases of venous insufficiency, patients have both obstruction of forward flow and backward leakage of the veins. The most common causes of venous insufficiency are previous cases of blood clots and varicose veins. PIPERINE, AN ALKALOID DERIVED FROM BLACK PEPPER, INCREASES SERUM RESPONSE OF BETA-CAROTENE DURING 14-DAYS OF ORAL BETA-CAROTENE SUPPLEMENTATION

The effectiveness of an extract from the fruit of black pepper, consisting of a minimum of 98% pure alkaloid piperine was evaluated for its ability to improve serum response of beta-carotene during oral supplementation using a double-blind, crossover study design. Subjects were randomly selected to ingest a daily beta-carotene dose (15 mg) either with 5 mg of piperine or placebo during each of two 14-day supplementation periods. Inter-subject variability in pre-supplementation serum beta-carotene levels was minimized by limiting the selection of volunteers to healthy adult males with fasting serum beta-carotene values < 20 mcg/dL. The results indicate that significantly greater increases (p < 0.0001) in serum beta-carotene occurred during supplementation with beta carotene plus piperine (49.8±9.6 mcg/dL vs. 30.9±5.4 mcg/dL) compared to beta-carotene plus placebo. Supplementation with beta-carotene plus piperine for 14 days produced a 60% greater increase in area under the serum beta- carotene curve (AUC) than was observed during supplementation with beta-carotene plus placebo. We suggest that the serum response during oral beta-carotene supplementation is improved through non-specific, thermogenic property(s) of piperine described in this paper as thermonutrient action.

An extract from the fruits of black pepper consisting of a minimum of 98% pure
piperine was evaluated in a clinical study using a double-blind design. The
relative bioavailability of 90 mg and 120 mg of coenzyme Q10
administered in a single dose experiment or in separate experiments for 14 and
21 days with placebo or with 5 mg of piperine was determined by comparing
measured changes in plasma concentration. The inter-subject variability was
minimized by limiting the selection of individuals to healthy adult male
volunteers with (pre-supplementation) fasting coenzyme Q10 values
between 0.30 and 0.60 mg/L. The results of a single dose study and the 14-day
study indicate smaller, but not significant, increases in plasma concentrations
of coenzyme Q10 in the control group compared to the group receiving
coenzyme Q10 with a supplement of piperine. Supplementation of 120
mg of coenzyme Q10 with piperine for 21 days produced a
statistically significant (p=0.0348), approximately 30% greater, area under the
plasma curve (AUC) than observed during supplementation with coenzyme Q10
plus placebo. It is postulated that the bioenhancing mechanism of piperine to
increase plasma levels of supplemental coenzyme Q10 is nonspecific
and possibly based on its description in the literature as a thermonutrient.

Pinus radiata - a sustainable source
Pinus radiata (also known as Monterey Pine) is grown sustainably throughout New Zealand in the pure soil, water and air of New Zealand's sub-alpine conditions, in the absence of pesticides and chemical fertilisers.
This tree has flourished in New Zealand and is the country's most dominant species, with more than 1.6 million hectares of planted forest.
Only radiata bark of a certain age, from a particular portion of the tree and free from contaminants, is accepted for extraction into Enzogenol.
The bark is peeled off logs freshly harvested from a specialist forest, transported immediately to the Enzogenol production facility, ground, thoroughly screened and then washed in pure water that has been de-ionised, filtered and cleared of all oxidants.

Down through the ages, trees have provided abundantly for humans. They have been a source of wood for shelter, transport, weapons of war and instruments of cultivation, not to mention fuel for cooking and heating.

Living Longer - a study
These studies aimed to determine the effects of Enzogenol on ageing, health and well-being.
Mice were used in the experiment because, like humans, they develop degenerative diseases as they age. Much of this degeneration is thought to be due to the action of free radicals attacking cells and tissues in the body, as well as interfering with regenerative processes.
These studies revealed that Enzogenol not only slowed down these degenerative processes in mice, it also helped the mice to live longer and healthier lives.
Toxicity - No acute or chronic toxicity of Enzogenol was evident in any of the trials.
Effects on general condition - Mice fed Enzogenol grew thicker, glossier fur; their eyes became brighter and their noses and tails pinker - a sure sign of healthy mice!
When asked to pick the healthiest looking mice, independent observers invariably identified the mice that were fed Enzogenol over the control groups.
Effects on longevity - If the mouse results serve as a good model for humans, then a normal dose of Enzogenol increases the number of people who survive to a given old age, X, by 1.825 times. In other words, trial results show that old age survival to age X may be nearly doubled due to Enzogenol.

On the basis of these mice trial results, two classes of effects were observed:
Alpha (a) Effects - Alpha effects are the long-term effects of scavenging or neutralizing of free radicals. These effects do not change the way that people feel. The antioxidant is taken in the expectation it will prevent disease, cancer, heart attacks etc. There is no obvious change - which could explain the longevity and reduced tumor incidence in the mice.
Beta (ß) Effects - Beta effects are noticed more quickly. These are the changes that you notice in health, psychological or emotional state, or that others notice in you. In this case the antioxidant is affecting metabolic processes with subsequent changes in physical, emotional or physiological state. Beta effects cause increased alertness, improved skin condition, less flu’s and colds, faster recovery from soft tissue damage, improvement in joint movements and an increased ability to cope with stress. These may be the more compelling reasons for taking a powerful antioxidant.

The pharmacological effects of anthocyanosides are mainly evident on microcirculation where different activities have been reported:
This activity is related to the ability of the anthocyanosides both to reduce capillary permeability and to increase capillary resistance, working through several different mechanisms of action:
- Stimulation of the biosynthesis of mucopolysaccharides, constituents of perivascular tissues, where are recognised as playing an important role in maintaining the integrity of microvessels.
- Inhibition of proteolytic enzymes, such as elastase, which are involved in the degradation of collagen and other components of the extravascular matrix of the smallest blood vessels.
- Antioxidant activity that gives protection to the integrity of microvessels against free radicals damages, particularly after ischemia - reperfusion injury in the microvascular network.
These effects prevent the formation and promote the regression of hard exudates (index of altered permeability of capillaries) and microhaemorrhages (index of altered resistance of capillaries) in some tissues (such as the retina) where the microvascular network is significantly compromised during very common pathologies (diabetes, arterial hypertension).
The vasoprotective action of the anthocyanosides has been shown using a large number of experimental models and in different parts of the body, including the Central Nervous System and the eye (7). This interesting ophthalmic property has been demonstrated in an experimental study where anthocyanosides were able to reduce the abnormal permeability of the vessels of the rabbit eye ciliary body
Activation of arteriolar vasomotion
Anthocyanosidic extract is reported to increase arteriolar vasomotion, the rhythmic variation in arteriolar diameter in the microvascular network. The stimulation of arteriolar vasomotion may improve microvascular blood flow, promoting the redistribution of blood flow and decreasing the formation of interstitial fluid may be beneficial in increasing the supply of nutrients to the tissues with a very active metabolism (such as the retina, involved in a continuous regeneration of rhodopsin) and reducing the effects of increased vascular permeability, including localized oedema.
Stimulation of regeneration rate of rhodopsin
Experimental studies have shown that anthocyanosides are able to improve the function, probably by increasing the regeneration rate of rhodopsin present on the retina. In fact, excessive stimulus of light over long hours (i.e. heavy exposure to computer or TV screens) can accelerate the decomposition of rhodopsin, resulting in the shortage of this pivotal substance for vision, giving rise to a decline in visual function.
The results of pharmacological tests on anthocyanosides have suggested further clinical studies to evaluate the efficacy of Mirtoselect® in the treatment of microvascular disorders in ophthalmology and phlebology. In addition, the potential ability of anthocyanosides in sight improvement has been investigated in adults and children suffering from ocular fatigue due to overuse of eyes.
Mirtoselect® in diabetic and hypertensive retinopathy
Ophthalmoscopic examinations, including fundus fluorescin angiography performed both before and after the study, showed a significant reduction of retinal microhaemorrhages and a regression of retinal localised oedema following treatment with bilberry extract (10,11). These findings confirm the vasoprotective properties of anthocyanosides from Vaccinium myrtillus by clinical investigations.

insufficiency of the lower limbs
Preclinical studies in animals on acute and chronic toxicity have shown that Mirtoselect® has an excellent safety and
tolerability profile. In dogs, a single oral dose of 3,000 mg/kg did not induce any sign of adverse effects as well as the oral administration of 80-320 mg/kg
daily over a period of six months (1).
The pharmacological and clinical findings suggest several clinical applications of Mirtoselect® in ophthalmology and phlebology:
· Treatment of fragility and altered permeability in the microvascular network of the retina associated with diabetes and arterial hypertension. The vasoprotective activity promotes the regression or disappearance of retinal microhaemorrhages and exudates, and as a result may help counteract the progress of retinopathy.
· Treatment of ocular fatigue due to excessive stimulus of light over long hours for example by heavy PC or TV monitor screen viewing or in myopic patients with a decline in visual function due to overuse of the eyes. In fact, anthocyanosides from Vaccinium myrtillus restore vision in a rapid manner accelerating resynthesis of rhodopsin present on the retina, a chromoprotein with a pivotal function in recognizing the things visually. In addition, Mirtoselect® increases the blood supply to the retina, apporting in this way a big amount of nutrients for its functionality.
· Treatment of symptoms associated with chronic venous insufficiency of the lower limbs. Mirtoselect® may also provide rapid relief of limb oedema, feeling of heaviness, cramp-like pain, thanks to the reduction of capillary permeability and a vasoprotective action.
COMPATIBILITY WITH OTHER NUTRITIONAL INGREDIENTS
Mirtoselect®'s anthocyanosides can be used to produce multicomponent formulations containing other active substances (i.e vitamin E, vitamin C, ß-carotene, flavonoids from other sources) for a synergic vasoprotective action on microcirculation. For restoring vision it may be interesting to associate anthocyanosides with agents that act synergically in accelerating the resynthesis of rhodopsin (i.e. ß-carotene or vitamin A).
Numerous studies have been conducted to identify the pharmacological activities of anthocyanosides from Vaccinium myrtillus berries and to assess their clinical efficacy. Most pivotal studies have been carried out with Mirtoselect®, a standardized bilberry fruit extract containing 36% anthocyanosides. Therefore, Mirtoselect® is the only bilberry extract with proven efficacy. Pharmacological studies have shown that Mirtoselect® is effective in increasing capillary resistance, likely assits in reducing abnormal vascular permeability and performing antioxidant activity at vascular level. These pharmacological findings have suggested a clinical application in the treatment of vascular disorders of the retina due to diabetes and hypertension (retinopathy) and in the management of symptoms associated with chronic venous insufficiency. Several randomized, double-blind and placebo-controlled clinical trials clearly suggest significant efficacy of Mirtoselect® in hundreds of patients treated. In addition, Mirtoselect® promotes the resynthesis of retinal pigments (rhodopsin) and there is clinical evidence that the intake of bilberry extract is useful for the recovery of visual acuity and relief of eye fatigue symptoms in case of eye overuse.
